We have developed this safe, online medical record and forms requesting process to save you time and energy.
Please have the following available before you begin:
For all requests
The name of your physician, their practice, and their physical address and/or email.
The physical address and/or email of the location you want your information sent.
Identification, such as a driver’s license, passport, or a government-issued non-driver identification card.
To ensure that your request process is uninterrupted, you will need the following pieces of information at your fingertips. Having all your information ready before proceeding will eliminate stress and frustration.

Use our secure portal to submit your request to access records
Once you have completed the steps in the requesting process, an RRS Medical certified specialists gathers and compiles your appropriate files. They ensure that correct records or forms are compiled correctly and made ready for delivery. When the above steps have been completed, you or the designated party will be notified and provided a PIN to securely access and download your documents.
